Requirements
- Copy of your Operating Authority (including any re-entitlements)
- Copy of your Satisfactory Safety letter from the DOT
- Copy of your C-TPAT Certification including SVI number
- Carrier Profile Form
- Broker Carrier contract
- Certificate of Insurance
- W-8 Form
