Insurance Requirements
Your insurance certificate must:
- List all subsidiaries or DBAs covered by the certificate provided
- Provide at least 30 days prior notice of cancellation or material modification of any policy
- Must include a modified cancellation clause; the wording "Endeavor to" and "but failure to mail such notice shall impose no obligation or liability of any kind upon the company" must be deleted from the certificate
- Show complete insurance carrier names as listed in the A.M. Best Property & Casualty Guide
The certificate of insurance must be completed in its entirety and signed. Binders are not acceptable. View a Sample Certificate in our Document Library.
Commercial Automobile Liability
$1,000,000 Combined Single Limits
Coverage must be placed with a carrier rated not less than B, V by A. M. Best. Must indicate coverage for any auto, or all owned, hired and non-owned autos.
Cargo Liability
$100,000 Per Trailer Or Conveyance
Coverage must be placed with a carrier rated not less than B, V by A. M. Best.
Workers' Compensation

Employers' Liability Coverage
- $100,000 each accident
- $100,000 disease - policy limit
- $100,000 disease - each employee
