Werner Enterprises offers a PPO (Preferred Provider Organization) for medical and dental insurance coverage. Blue Cross Blue Shield of Nebraska (BCBS) provides our health insurance. Employees may elect medical insurance only, or medical and dental insurance. Dental insurance is not available without medical insurance coverage.
Full-time employees who work at least 36-hours a week are eligible for medical and dental insurance. Spouses and children (biological or adoptive) until the age of 19 years, or 23 years old if a full-time college student, are eligible if the employee elects coverage. If you do not enroll within the first 30 days of hire you may enroll during open enrollment which is held in November of each year or if there is a qualifying event.
You may only add dependents to your health and dental insurance during initial enrollment or if one of the following events occur and you provide the Benefits Office with proof of a qualifying event within 30 days of the event: birth or adoption of child, marriage, divorce, or if you, your spouse or children lose coverage from another employer provided plan.
Following acceptance of the insurance Application, the employee's coverage becomes effective the first of the month following 30 days of employment (example: you started on Feb 16th, coverage would begin on April 1st).

The Company will pay a portion of the cost of the coverage and the employee will pay a portion through payroll deduction.
The Plan provides comprehensive hospital and major medical insurance coverage. Participants may choose to use a preferred provider in the network and pay lower costs for medical services, deductibles and coinsurance. If you choose to use an out-of-network provider you will be billed at the higher out-of-network rate. As a part of the health benefit, prescription drug coverage is provided through a nationwide pharmacy provider and a prescription drug card. The employee is responsible for the copayment or coinsurance, depending on the prescription drug covered at the time of purchase.
The employee's contribution for health and health and dental insurance will be deducted from the employees's paycheck on a pre-tax basis. This pre-tax advantage is made possible by Section 125 of the Internal Revenue Code.
The information provided herein regarding insurance coverage is intended as a summary only. For detailed information, please refer to the Summary Plan Description (SPD) which can be obtained from the Benefits Department.
