Coverage:
Superior Vision offers a nationwide network of providers including many convenient and one-hour locations such as: Walmart, LensCrafters, Pearle, Sears, JC Penney's, EyeMasters, and many other independent practitioners.
Eligibility
Contractors who work at least 36-hours a week are eligible for vision coverage. If you do not enroll within the first 30 days of hire as a contractor you may enroll during open enrollment which is held in November of each year.
Coverage
Following acceptance of the insurance Application, the contractor's coverage becomes effective the first of the month following 30 days as a contractor (example: you started on Feb 16th, coverage would begin on April 1st).

Plan Designs
Two Plan Options are available, both with the same low copays: Eye Exams - copay $10.00; Frames - copay $25.00 Basic Plan - One eye exam, frame and lenses benefit is available once each 12 rolling months; frame allowance is available once each 24 rolling months.
Enhanced Plan - One eye exam, frame and lenses benefit are available once each 12 rolling months. *The contact lens benefit can be used in lieu of eyeglass lenses and frames benefit.
A 20% discount off the usual and customary surgical fees for radial keratotomy (RK), photo-refractive keratotomy (PRK) and LASIK.
